RESUMO

En la actualidad se cuenta con escasos reportes de casos clínicos de pacientes con metástasis coroidea sintomática como la presentación inicial de cáncer de pulmón. En este reporte describimos el caso de una paciente con adenocarcinoma de pulmón, la cual fue diagnosticada por síntomas iniciales oculares consistentes en pérdida visual progresiva y presencia de escotomas en ambos ojos, por lo que se le realizaron exámenes adicionales que llevaron al diagnóstico de cáncer de pulmón con metástasis coroidea. El pronóstico de estos pacientes es pobre, en la mayoría de los casos se trata de tumores en estadios avanzados de la enfermedad.


Currently there are few clinical case reports of patients with symptomatic choroidal metastasis as the initial presentation of lung cancer. In this report, we describe the case of a female patient with lung adenocarcinoma, who was diagnosed due to initial ocular symptoms consisting of progressive visual loss and the presence of scotomas in both eyes, so additional tests were performed leading to the diagnosis of lung cancer with choroidal metastasis.

RESUMO

Culantro de monte (Eryngium foetidum L.) is a biennial herb of tropical origin very frequent in America, Africa and the Caribbean. With the arrival of Europeans in the fifteenth century, it managed to be distributed to various locations in Asia and Europe. The present study was carried out in the San Antonio de Padua community in the Province of Los Ríos - Ecuador to collect information, through interviews with the residents, regarding the nomenclature and uses of this aromatic herb. It was found that 52% of the interviewees mention a nutritional use, especially as a condiment for fish soup, buns and stews, while 26% say they use this plant for medicinal purposes in infusions to cure stomach and joint pain. The variety of metabolites that it possesses, such as eryngial (E-2-Dodecenal), extends its range of applications in modern medicine. We concluded that the transfer of ethnobotanical information still remains in the community, but the young generations are unaware of several of this plant's applications.


El culantro de monte (Eryngium foetidum L.) pertenece a la familia Apiaceae, es una hierba terrestre bienal de origen tropical muy frecuente en América, África y el Caribe, con la llegada de los europeos en el siglo XV, logró distribuirse a varias regiones de Asia y Europa. El presente estudio se realizó en la comunidad San Antonio de Padua en la provincia de Los Ríos ­ Ecuador, tuvo como objetivo recopilar información, mediante entrevistas escritas a los pobladores, referente a la nomenclatura y usos de esta hierba. Se encontró que un 52% de los entrevistados le da un uso culinario para preparar sopas de pescado, sancochos, bollos y estofados, mientras que un 26% dice utilizar esta planta con fines medicinales en infusiones para tratar en el ser humano dolores estomacales y dolores de las articulaciones. La variedad de metabolitos que posee, como el eryngial (E-2-Dodecenal), amplía su rango de aplicaciones en la medicina moderna. Se concluye que el flujo de información etnobotánica se mantiene en la comunidad, pero las nuevas generaciones desconocen las varias aplicaciones que tiene esta especie.

Objetivo Estimar la percepción de los pacientes sobre la atención médica en el primer nivel de atención. Metodología Se aplicó una encuesta telefónica a pacientes atendidos en dos meses diferentes del 2012, indagando por variables sociodemográficas, relaciones médico-paciente y acerca del proceso de atención médica. Resultados Se encuestaron 804 pacientes. El tiempo promedio de acceso a la atención fue de 9,6 días. El 78% refiere haber podido contar todo lo que sentía al médico, el 60% que el médico le explicó lo que tenía y, uno de cuatro,que indagó por su familia. El 30% sintió alivio completo luego de la atención médica. La calificación promedio de la atención médica fue de 7,9 (DE ±1,7). Las variables relacionadas con las calificaciones más altas fueron: Poder contarle todo al médico (OR 7,5IC 95% 1,8-31), ser examinado (OR7,5 IC 95% 1,5-38,5, explicarle quétiene (OR 5,2IC 95% 1,8-15), preguntar por la familia (OR 5,8 IC 95% 2,1-16,1)y haberlo atendido antes (OR 3,5 IC 95% 1,4-8,6). Conclusiones La comunicación extensa con el paciente es tan importante como el enfrentar la enfermedad en el acto médico.(AU)
